Are you an Oregon SNAP-Ed Educator?
This is a revised version of the CTS adult intercept tool, updated for 2025 - 2026 with new questions about kitchen tools/equipment participants may be using for meal preparation and cooking in their households. Two formats are included for English and Spanish language surveys: the 2-page version includes the original format and can be printed front/back and completed surveys scanned or mailed back to campus. The 3-page format includes larger images and font sizes for the cooking tools questions on pages 2 and 3, for better accessibility. Please staple completed paper surveys and scan or mail to campus for processing.
